Who needs our services?

Co-op Provider specializes in providing services to adults with mental health and intellectual/ developmental disabilities, both in-home and out of home, including Community Living Arrangements (CLA) and Lifesharing.Many seniors don’t realize that there is an alternative to moving out of their homes into an assisted living facility, nursing home or another kind of institution. Most people prefer to remain at home for as long as possible - we can help! oIn fact, there are more and more people every day who can benefit from our services.

 

What do we do?

Our trained staff provides the following services:  Habilitation, Companionship, Behavioral Therapy, Personal Care, Respite, Supported Employment, Community Inclusion/Integration. Depending on our clients’ needs, we can also provide light housekeeping, laundry, meal planning and preparation, running errands, assistance with activities of daily living such as personal hygiene and grooming assistance. Our services provide needed social interaction to reduce isolation and improve the quality of our clients’ lives.

Our services are distinct, individualized and consumer-driven, and are designed to empower people with special needs and enhance their quality of life.  By utilizing positive and best evidence-based practices, we help our consumers meet their goals and objectives for the best possible outcomes.  We work collaboratively with the consumer so they can learn best about themselves and their world through real life experiences.  By offering our clients choices and an array of opportunities, it helps them achieve greater independence.  We continually strive to provide an effective array of accessible services and supports that are flexible, innovative and person-centered.

 

About Us

Co-op Provider is an approved Provider by the Department of Human Services in the Commonwealth of Pennsylvania, Qualified by the Office of Developmental Programs (ODP) and the Bureau of Autism (BAS) for the Consolidated and Personal/Family Direct Services (P/FDS) waiver programs. We are subjected to 55 PA Code 51 and 52 rules and regulations. Provider qualifications and verification requirements are continually being updated.  We also are governed by federal, state and local requirements.  Co-op Provider must meet applicable waiver amendments and have valid MA provider agreements that are monitored by the various departments on a regular basis. Our licensure and certifications as an agency that provides Home & Community Services is the key to our success.

    Our programs are designed with inclusiveness in mind, and are client/consumer/participant driven in order to enhance the quality of lives. Services provide individuals the necessary skills to live more independently. We enable our clients to participate in the community to give them opportunities to develop mind, body and spirit. We believe that all people with disabilities are productive members of the community. Co-op is a cooperative in the sense that our family, friends, agencies and community work together by pooling our skills, labor and resources to create a better place for our clients with disabilities. It is imperative that we all cooperate together.

     

    Co-op Provider is at the forefront of information technology in our industry, and we are working to become virtually a paperless office. Co-op Provider can provide services by relatives, legal guardians and legally responsible individuals as staffpersons.

     

  • Our Leadership

     

    Cathy Stein

    Founder/President

     

    My name is Cathryn Stein (Cathy), and I am the CEO for Co-op Provider Services. I was raised in Seattle, Washington and am a graduate of Washington State University and attended Grad School at the University of San Diego. I live in Chester Springs, PA. with my husband and we have two sons. Our eldest is a graduate of Penn State University and is a Lieutenant in the United States Army. Our youngest is dual-diagnosed and is working at Handicrafters. I have lived over half of my life on the East Coast, and have come to embrace the heritage, culture, and landscape of this community. I have over 25 years experience working as a paralegal for some of the top law firms and Fortune 500 companies from coast to coast. Working cooperatively with the vast resources and acquaintances I’ve developed over the years has proved that together we can change and enhance the lives of individuals with disabilities. With this belief I created Co-op Provider Service Network. I am very active in the equestrian world. I was on the management board of our local Pennsylvania Special Olympics, where I was a certified Equestrian coach and Alpine coach. I currently participate in Special Olympics ice skating and skiing. My interests include: camping, fishing, hiking and even doing living history as a Civil War Reenactor. I was a Merit badge counselor for the Boy Scouts of America. With all of our contacts, resources and fierce determination we can network and make miracles happen. I truly believe in the human spirit. Co-OpProvider.com was founded on these principles.

     

     

    Carol Koehler

    Lead Clinical Coordinator

     

    I am Carol Koehler and am the Lead Clinical Coordinator for Co-op and a licensed social worker and licensed behavior specialist in Pennsylvania. I received a Masters in Social Work from West Chester University and a Masters in Management from Penn State. I am pursuing a Masters in Applied Behavior Analysis with an Emphasis in Autism in anticipation of taking the Board Certified Behavior Analyst (BCBA) Exam. I have had a passion for working with clients with developmental differences for the last 10 years, first as Therapeutic Support Staff working with children and since 2011 as a Behavior Analyst supporting adults on the autism spectrum and with other developmental differences. In addition, I have a family member diagnosed with autism so I understand firsthand that each family has unique challenges as well as their own important strengths and perspectives on meeting those challenges. I understand and value each family’s commitment to pursuing and maintaining high quality services that work comfortably for your family. My clinical approach is to meet clients where they are at and integrate a strength based perspective when helping our clients meet their goals. I love working with our clients and I have found that joining my therapeutic skills with evidence based behavioral practices offers an extremely effective skill set for working collaboratively with families and other team members to assess strengths and needs, develop and implement comprehensive plan, and provide ongoing monitoring and evaluation to support each client’s ongoing progress.

     

     

     

    Crysta

    Chief Financial Officer

     

    I’ve been with Co-op since 2011. I take care of everything related to accounting and collaborate with other administrators to create and improve our internal systems. Prior to joining Co-op I spent 17 years doing much the same for a wonderful individual and his two companies. Always striving to assist and serve our clients and staff to the best of my abilities. My interests include teaching riders and training horses. I have 13 horses currently under my care and enjoy coaching both high school and middle school riding teams. Skiing and watercolor painting are additional passions.

     

     

    Sharon Halteman

    Director of Human Resources

     

    My name is Sharon Halteman and I am the Director of Human Resources for Co-opprovider. I received my B.A. in Sociology from Gwynedd Mercy University. As Human Resources Director I not only manage administrative duties, recordkeeping and recruitment, but I also have 20 years of experience in the home health care field. In the past I have worked directly with adults and children with disabilities such as autism, down syndrome and intellectual disabilities. Currently I also provide respite services in my home.

     

     

     

    Jennifer Shaffer

    Administrative Director

     

    My name is Jennifer Shaffer, and I am the Administrative Director for Co-op Provider. I received my B.A. in English from West Chester University, and have a diverse background, having done copywriting for several ad agencies, marketing and writing for a semiconductor company, book editing for a publisher, and freelance reporting and writing for a group of newspapers in Clearwater, FL. I was more recently the Office Manager for a busy medical practice, and prior to becoming Administrative Director at Co-op was a direct support professional to consumers with disabilities such as spina bifida, down syndrome and autism. In addition to conducting one-on-one company orientation sessions with new staff, I attend ISP meetings and work closely with supports coordinators to coordinate our clients’ services. Having a family member with autism has given me personal experience on how this diagnosis touches lives.

     

     

     

    Cindy Eves

    Nurse

     

    My name is Cindy Eves. I grew up in Montgomery County, PA. I currently live in Berks County, PA. I live with my husband, son age 13 and daughter age 11. We are a very active family enjoying swimming, hiking, skiing and snowboarding. I graduated nursing school in 1997 with a BSN from Widener University. I have 18 years experience in nursing. I was employed at Manor Care, Montgomery County Geriatric Center, Phoenixville Hospital, and Pottstown Memorial Medical Center where I still work. I received excellent commendations from all of them. I became a nurse because I wanted to help people who are in need. Throughout my career I have learned that I have a great aptitude for the nursing profession. I have learned from the people I helped and that learning has made me the person I am today. I look forward to serving you through Co-op provider.
